有關111-1學期TA證書與TA獎勵金申請方式，請參閱以下說明。

【TA證書申請】

完成 2必修+ 2選修 = 4門課程的同學，即日起至 https://b008.ndhu.edu.tw/files/87-1010-2370.php申請證書，並上傳符合2+2課程之學習護照以示證明。因證書採用批次印製，12/30 (五)之前申請同學於112/1/9〈一〉起至中心領取TA證書。若已有證書且本學期擔任TA者，須選擇一門實體或線上課程進修回流。

申請證書注意事項：

1. 跨學期修課可列入計算
2. 其中實體(可含直播)至少2門
3. 學術倫理可抵免線上必或選修課程1門

【優良TA獎勵金申請】

欲申請優良TA的同學，若您是擔任本學期〈111-1〉的教學助理，並持有TA證書，且授課教師同意卓越中心到班或線上施測《TA滿意度調查問卷》，申請人請於12/5〈一〉-12/9〈五〉攜帶相關申請表至教學卓越中心〈行政大樓411）辦理申請，逾期不受理，趕緊把握申請機會，我們將擇優選拔15名優良TA〈可獲優良TA獎狀一式與三千元獎勵金〉！！若尚未領取TA證書可用學習護照代替證明。

【111-1 Application for TA certificate and TA Excellence Awards】

TAs have to complete 2 compulsory courses and 2 elective courses this semester, then you can apply for the TA certificate and receive it after Jan, 9 at the administration Bldg. Room 411(Center for Teaching Excellence).
